Clinical Question Do preoperative pain medications reduce pain in patients with endodontic treatment compared with patients with no postoperative pain?
Clinical Bottom Line 200mg Ibuprofen or 20mg of tenoxicam reduces postoperative pain at 6 hours after Endodontic treatment but no significant difference at 12,24,48 and 72 hours.

Key results48 patients (age between 18-52) with no significant medical problems were divided in to 3 groups. Patient consented to double blinded oral administration of either 200mg Ibuprofen, 20mg of tenoxicam or sugar placebo before endodontic treatment. The three test groups were randomized by block randomization method. Patients were treated only by one graduate endodontic resident. Pain was determine using the visual analog scale. Data were statistically analyzed by the two–way ANOVA test and Tukey HSD post hoc test. Postoperative pain in the 6 hour period with Ibuprofen and Tenoxicam groups where significantly lower than the control group (p=0.000). Conversely, there were no significant difference between the 3 groups at 12,24,48,72 hours.

The Evidence
Validity: This study is provided with a high Level of Evidence. Good sample size. Perspective: The study did not show a definitive diagnosis of the endodontically treated teeth which would have been useful to know if there is a difference between vital and non vital teeth.
Applicability The clinician is able to provide the patient with prophylactic medication prior to endodontic treatment to reduce postoperative pain but it will not be effective after 6 hours of treatment.
